[subject] + 从来 + 不 + [verb phrase]
[subject] + 从来 + 没(有) + [verb] + 过
[subject] + 从来 + 都 + [verb phrase]
从来 means "at any time, ever". It almost always comes with a negative, and together they mean "never". The negative decides which "never" you get.
从来不 is about habit or choice: something someone doesn't do, as a rule. 从来没 says something has never happened, and the verb takes 过.
Without a negative, 从来 needs 都 (or 就) after it, and the meaning turns to "always, all along".
从来 goes after the subject, before the negative. It doesn't open a sentence the way "never" can in English.
