[topic] + 以 + [noun] + 为准
请 / 一律 / 均 + 以 + [noun] + 为准
以 X 为准 says X is the version that counts when two sources disagree. It belongs to notices, contracts, timetables and price tags. 为 is read wéi here.
The thing being settled usually comes first as a topic, and X is a noun phrase naming a document, a measurement or an authority: 通知, 合同, 官网, 实际情况, 中文版本.
A notice that tells the reader where to check puts 请 before 以. 一律 or 均 says the rule has no exceptions.
为准 is fixed. Learners swap in 是 or drop 以, or put the noun after 为准 as if it were a verb.
以…为主 and 以…为例 use the same frame with a different second word; they mean "mainly X" and "taking X as an example".
