The structure is Number + Counter + おきに, meaning "at intervals of ~" or "every ~."
When used with 一 + counter, おきに often translates as "every other", since it describes a gap of one unit between occurrences.
Similarly, ふつか二日おきに means "every third day" (with a two-day gap between each occurrence), にしゅうかん二週間おきに means "every third week," and so on.
A related expression is ごとに, which also means "every." The key difference is that おきに emphasizes the interval (gap) between occurrences, while ごとに emphasizes each unit as a whole. For example, さん三じかん時間おきに means "at three-hour intervals" (with three hours in between), while さん三じかん時間ごとに means "every three hours" – in practice, these often overlap, but with smaller numbers the distinction matters more: いちにち一日おきに (every other day) vs. いちにち一日ごとに (every single day).
