この上ない means "there is nothing above this": the greatest, the utmost. It stands in front of a noun; この上なく, its adverb form, stands in front of an adjective or a verb.
この上ない goes mostly with good things: joy, honour, happiness. このうえ上もない is the same word with も added for emphasis.
ことこの上ない comes after an adjective: an い-adjective as it is, a な-adjective with な. It means "as X as anything could be", and it most often goes with something unpleasant.
All three forms are literary. They suit writing, speeches and formal thanks. In conversation, さいこう最高の, これいじょう以上ない or ほんとうに本当に say the same thing.
